When coming for your first appointment please plan to arrive 30 minutes early to complete the necessary paperwork. If you decide to print out the paperwork to complete at home; please plan to arrive 15 minutes early for the front office staff to review your forms and verify that everything is in order for your first visit. Bring your photo id and insurance card to your first appointment.
Our appointments typically run on the hour and run for a clinical hour (50 minutes).
